I dunno if you played Ninja Gaiden 2, but you're getting killed in seconds on highest difficulties because how fast and relentless enemies. so It's just requires to get sense of timing to get past them, instead of simple dodge at the right moment. That kinda separates games holding your hands from games that require practice .
Even in the games you listed there is still a clear and identifiable warning before attacks.
And you don't need attacks without warning to have a difficult game.
Take the harpys and Dreamrunners in DmC for example; they have the fastest attacks in that game. But they both have clear warnings before they do attack.
