Yeah I got a message from her on Facebook about it, I don't understand it because according to the mod CP, she's not banned, I honestly have no clue what's happened, she's not banned on the user's list, the mod control panel, nothin'.

Possible, if not highly probable, this site has never implemented a mandatory survey feature, at least not to my knowledge, I'd say that's the most likely explanation.

Improbable, if she forgot her password she'd have to forget it five times, and even then, it'd only stop her logging in for fifteen minutes, not ban her outright.
