It's not an actual school uniform. It just looks like one because of the shirt and combat skirt being plaid.
in the manga she did have a school girl uniform though.

It's been a year between events of manga and DMC3, Dante says so, so the answer is most likely yes. However that does not with certainty answer if she is still a minor or fresh adult.

I can't help but think she's seventeen. In Japan, that's the age that's second to be fetished over with ages 14-16 in first.
