DmC lost souls and every hero has a weakness glitch

I don't know if it's only happening to me or other people but there is one lost soul in the furnace of souls level that is not showing up it's near the gold door and the trophy every hero has a weakness is glitching i've done the mission multiple times and on different difficulties and it's not showing up I hope Ninja Theory can fix this soon

Doesn't really help, but I didn't have any issues completing both.

With the achievement, you probably got hit and didn't realize it. Trust me, unless you do it on heaven or hell its really hard to tell at times.
